Sciatica Definition
Sciatica refers to sciatic nerve lesion which causes pain symptom along the sciatic nerve pathway from the lower back, buttock, and down through the lateral thigh, the lower leg and foot.
Sciatic nerve is the main nerve trunk to control the lower body. The pain distributes along the sciatic nerve pathway and within its region of buttock, lateral thigh, lower leg and foot.
Sciatica Causes
For unknown aetiology, primordial sciatica (also known as ischiatitis) is uncommon in the larynx.
Due to compression or irritation caused by neighboring lesions, secondary sciatica can be divided into root sciatica and trunk sciatica depending on whether the compressed position is nerve root or trunk. Root sciatica is more common which is mainly caused from lumbar disc herniation. Intraspinal tumor, vertebral metastasis, tuberculosis of lumbar spine and lumbar spinal stenosis may also cause root sciatica. Sacro-iliitis, pelvic tumor, gravid uterine compression, coxitis, buttock trauma and diabetes mellitus are etiologies of trunk sciatica.
Kinesitherapy of Sciatica
Sciatica usually affects one side of the lower body and is made worse by coughing or a bowel movement at night. There are two types of sciatica, one is primordial sciatica, and the other is secondary sciatica. Primordial sciatica results from lesion of ischiatitis. Secondary sciatica is commonly caused by lumbar disc herniation, lumbar vertebrae, soft tissue injury of the lower back and buttock, and lesions inside the pelvic cavity and canalis spinalis. Besides avoiding catching a cold, patients should do exercises that target the muscles of the lower back and legs, from which they can benefit a lot.
First, swing the leg to the left and right alternately. Stand straight with hands on the wall, and then swing the leg to the left and right alternately. Ensure that the foot would not touch the floor when swinging.
Second, raise the legs alternately. Keep the lying position while raising legs alternately after stretching. Legs could be raised much higher through frequent exercise.
Third, exercise of bicycle riding. Keep the lying position while performing leg-pedaling like bicycle riding. The pedaling range could be gradually extended during the exercise.
Fourth, sit straight and raise the leg. Keep the legs tightly or insert a book between them. Make sure that knees are straight and heels on the floor. Put hands on both sides of the stool, and raise the leg above the position of the navel then put the legs down. At the beginning, patients might not raise their legs very high. After sustained exercise, they would achieve it.
Fifth, massage legs while sitting. Raise the tiptoe and make heels on the floor. Put hands on the thigh, and then bend forward. Meanwhile, two hands massage legs from the thigh to feet. Sustained exercise would bring up good results.
Sixth, jump while crouching. Put hands on both sides of the stool, bend the left knee and try to straighten the right leg to the right, and then do it alternately in the different direction. Prevention Knowledge of Sciatica
Many sciatica patients can still clearly describe that the morbidity was related to a sudden lumbar strain, for instance, when lifting and carrying heavy objects, a long time bending or after falling. Therefore, in order to avoid lumbar strain, warm-up is very necessary before doing heavy work in a sudden. The incidence of sciatica could be reduced after strengthening exercise on psoas muscles and improving the damp living environment. Acute sciatica patients need to seek medical advice timely. Curative effect is usually good if resting on bed and responding well to treatment.
Misunderstanding on Sciatica
Many medical instruction manuals, advertisements and science readings usually consider sciatica as a disease like lumbar disc herniation, lumbar spinal stenosis, acute sciatica strain and chronic lumbar strain. However, orthopedic professionals think it is necessary to correct this inexact and misleading understanding for sciatica.
The sciatic nerve is the longest nerve in human body; it runs from the root of lumbar spinal cord, through piriformis of the buttock, down the leg and foot, with the function of commanding muscular movement and conducting skin sensations. Normally one has two sciatic nerves on each lateral side of his legs. Among five lumbar vertebras, the last two lumbar vertebras are easily to become cataplasia and aging for burdening or moving too much.
Chronic and acute lumbar strain may cause the lumbar disc herniated, which makes the sciatic nerve compressed and leads to a series of pathological changes such as congestion, dropsy and adhesion. Sciatica pain starts from the herniated lumbar disc, radiating through the buttock and the back of the thigh, down the lower leg and foot, with sensation of numbness sometimes. Pain would be made worse by coughing. Just as fever, which is related to cold, encephalitis or meningitis, sciatica is only a symptom caused by pathological changes like lumbar disc herniation or lumbar spinal stenosis.
Among many causes of sciatica, lumbar disc herniation is the most common one, especially herniation of the last two discs or between the last disc and sacrum. In most cases, sciatica is probably lumbar disc herniation. In orthopedic department, we have found there are many diseases which could also cause sciatica besides lumbar disc herniation, such as lumbar spinal stenosis, lumbar spondylolisthesis, pyriformis syndrome, ankylosing spondylitis and intraspinal tumor, ect. Sometimes, non-professional orthopedists hurriedly make a diagnosis for sciatica patients and give a blind treatment without finding out the deeper pathology. Such a practice is not scientific and responsible.
Therefore, orthopedists should give a detailed physical examination to patients who go to the orthopedic department, with X ray film, CT and MRI (Magnetic resonance imaging) as assistance. Only by making a rational diagnosis for pathology can orthopedists give the symptomatic treatment, which will indeed eliminate patient’s pain. Jointly developed by Japanese expert team, high technology product (Lumbar Spine Recovery) has made an incredible breakthrough on sciatica cure and fundamentally solved the problem that sciatica is hard to recover and easily to recur. It is honored as “a major surgery without operating” by the medical community.
However, it should be pointed out that the symptom of lumbar disc herniation is not completely shown as sciatica. There are five lumbar vertebras in human body, with five corresponding lumbar nerve roots. Sciatic nerve consists of part of the fourth and the fifth lumbar nerve root, and the first, second and third sacrum nerve root. However, the second and third lumbar nerve root and part of the fourth one only take part in the composition of femoral nerve which is distributed in the anterior thigh. Compressed symptom of femoral nerve would be caused by the first two discs, the second and the third disc and the third to the fourth disc herniation. Patients usually feel weak to raise legs and have abnormal sensitivity in the anterior or inside skin of the thigh. Patients who suffer from sciatica caused by the fifth disc and the first sacrum herniation may never experienced sciatica pain. Some people just feel the lower back pain in the early or the whole occurrent stage; some intermittently hobble for miles, feeling sour, swell up and powerless. Only after a rest or crouching for a while can they continue to walk on. Some just feel cold of the limb, but those with severe sciatica may cause paralysis or gatism.
Sciatica Complications
Sciatica usually appears as complication of other disease, for instance, lumbar disc herniation, ect.
As a primary disease, patients with severe conditions might be disabled.
Sciatica Diet Therapy
The following diets are available for therapy:
1. Boil a pig’s tail with 25g Chuanduan and 30g Ducu, and eat after favored.
2. Stew an egg with 15g Sangjisheng and eat after wards.
3. Drink the soup after stewing a 500g weight young chicken and 6g Laosangzhi.
Supplementary Treatment for Sciatica
Adopt ice therapy to the affected part for 30 to 60 minutes several times a day when the pain occurs. After two or three day’s therapy, use hot-water bag to make fomentation in the same way, or take indomethacin or other OCT analgesic.
Make fomentation on the back or buttock with hot towel or hot salt wrapped by cloth. The temperature should not be too high, just take a sensation of comfort as appropriate
Attention For Sciatica Treatment
Rest on hard bed and keep on doing gymnastic exercise on it.
Strike a balance between work and rest, have a regular life and take part in some physical exercise properly.
Protect the lower back and affected extremity after exercise, change wet underwear in the prevention of dried by the body itself. It’s not appropriate to take a bath when sweating in case of catching a cold or wind.
During the period of acute sciatica never lift heavy objects over 10 pounds weight or raise them relying on the exertion of the leg, buttock or back. Pushing is much better than pulling.
In order to prevent the sciatic nerve traction and to relieve the pain, patients usually adopt a special sleeping position, for instance, they prefer to lie on the healthy side, with buttock and knee of the affected limb slightly bended. They take the healthy buttock as the center of gravity while sitting and take the healthy leg as the center of gravity while standing. When lifting heavy objects, they slightly bend the knee of the affected limb, which will cause scoliosis and mostly curve to the affected side as time goes by. Any experiment of sciatica nerve traction would induce or worsen the pain. It’s obvious to find the compressed pain along various points of the sciatica nerve pathway, such as near the lumbar spine, at the lateral ankle, below the capitulum fibulae, and in the centre of the sole of the foot. Besides feeling pain, patients would have numbness and tingling in lateral lower leg and on the back of the foot. Long time weakness in the muscle of the back thigh and the lower leg would cause sight muscular dystrophy.
For secondary sciatica, etiology must be found out in the first place. Patients should make sure whether they have tuberculosis, trauma, tumor and any other medical history or not. Besides taking a complete physical examination, women especially need to do the gynaecological check-up in order to find the primary lesion. X ray film has made a great contribution to approach the etiology. For the minorities, lumbar puncture and radiography examination should be taken into consideration if necessary. The first step for treatment of secondary sciatica is to eliminate etiology, such as diminishing inflammation and doing a surgery to remove the tumor. Acute sciatica patients should rest on hard bed. Sedative and analgesic might be properly given to relieve the severe pain. In the early stage of ischiatitis, analgesic is adoptable, such as piroxicam and indomethacin. Adrenal cortical hormone could be taken for a short period. B vitamins are helpful for both oral and parenteral administration. Keep the lower limb warm, and adopt supplementary treatment like acupuncture, physical therapy and massage. Patients suffer from severe pain in the first 5 to 10 days at the beginning, after 6 to 8 weeks the pain subsided and will gradually recover.
